You must use the content on accessible digital media (the “Work”) in compliance with the following terms:

• 1 - To take advantage of the DONA service:

a) You must have been diagnosed with a sensory, physical or neurological disability, as confirmed by a physician, and you must be referred to Copibec by your Educational Institution.


b) You must be duly enrolled in an Educational Institution at the college or university level.


• 2 - Only you can use the Work.
• 3 - You cannot sell, lend or reproduce the Work or authorize anyone else to take those actions.
• 4 - The Work may be downloaded and saved if necessary for the purpose of using assistive software.

Price

• 5 - If you do not need the paper-format version of the Work in addition to its accessible digital version, you are not required to purchase the paper-format version. To use the accessible digital version, you must pay Copibec the service charge as well as the same price charged for the paper-format version, as posted on the publisher’s website, for each copy of the Work.
• 6 - If you need both the paper-format publication AND the accessible digital version of the Work:

a) By completing the relevant section of the request form and signing this document, you certify to Copibec that you have already purchased a copy of the paper-format version of each Work.


b) To deliver an accessible version of a textbook, each publisher may, at its discretion, decide whether or not to charge 50% of the price of the paper-format version of the Work, as posted on the publisher’s website, up to a maximum of $50 per Work. If applicable, the student must pay the price set by the publisher for each copy of the Work, plus the service charge.


• 7 - Service charge: You must pay the $6 service charge per title ordered, regardless of the number of copies (e.g. 20 or 3 copies of a title = $6 service charge. But 1 copy each of 3 different titles = $18 service charge).

Within 48 working hours after receiving your signed form, Copibec’s finance team will send you an email so that you can pay your invoice by credit card. Once the payment has been made, you will receive an email containing a link to download the document.
